Maid Application Requirements

(A) Requested Documents

1.  Two (2) pieces of Passport Size Photographs for each Husband and Wife

2.  Two (2) photocopy of I/C for each Husband and Wife

3.  Photocopy of Certificate of Marriage

4.  Photocopy of Certificate for Children age below 15

5.  Original Letter of certification from Doctor for take care the old folk

6.  NRIC of old folk (sick family member / paralysis patient)

7.  Photocopy of birth certificate for Employer or Employer's Wife (old folk)

8.  Photocopy of Electricity bill / water bill / telephone bill (latest month)

 

Optional

9.  Original Letter of Cancellation from Overseas Embassy

     (If previous maid cancelled by overseas Agency)

10.  Photocopy Receipt of Compound Payment for house maid that absconded

11.  Photocopy of Check out Memo for previously maid

12.  Photos of Applicant’s residence

       (street name, house no.,outlook, living room, kitchen & etc

       as per requirements of Immigration)

B) Requisition of House Maid for Employer under Employment

1.  Original Letter of Confirmation of Employment from Employer

2.  Original Monthly Salary Slip for latest three (3) months

3.  Photocopy Statement for EPF (latest month)

4.  Photocopy Form B / BE and income tax payment slip (latest)

5.  Photocopy Statement of PCB with payment slip (latest)

C) Requisition of House Maid for Self-Employed Employment

1.  Photocopy Form B / BE and income tax payment slip (latest)

2.  Photocopy Statement of EPF (latest)

3.  Photocopy Statement of PCB with payment slip (latest)
